I,______________________________________________________________________(the student/client), have carefully read the below stated arrangements set forth and determined by A in J FORTUNE. Stated arrangements read(on the website) and fully understood:
The A in J FORTUNE Sapporo Homestay Refund/Money Back Policy.
The A in J FORTUNE 8 or 15 day course programs & costs.
The A in J FORTUNE signed agreement (this paper).
I,______________________________________________________________________(the student/client), agrees to and DOES HEREBY INDEMNIFY (hold harmless) A in J FORTUNE, its president/owner, company staff employees and the Sapporo, Japan, homestay family(or families), from ANY liability (accidental injury, death, sickness, theft and/or damage to luggage, etc.,ANY liability whatsoever) while I,________________________________________________________ (the student/client) am in Sapporo, Japan, and also while physically present in the homestay family home in Sapporo, Japan. By signing my name in each of the blank spaces above and below and dating this agreement; SIGNIFIES that I,________________________________________________________ (the student/client) will adhere to(fully understand) the stated arrangements and will COMPLETELY INDEMNIFY (hold harmless) A in J FORTUNE and the Sapporo, Japan, homestay family(families) of any unforeseen mishap/problem while I,________________________________________________________ (the student/client) am engaged on the 8 or 15 day (or longer arranged program) in Sapporo, Hokkaido, Japan. Student/Client (Written/Signed in the English Language Only).
